- RECHECK ALL forms for accuracy. Each online learner is responsible for providing their correct information (first name, last name, address, social security, and date of birth). If you do not have a US social security number, enter “000000000” or the number assigned by NYSED to applicants without US social security numbers. Information on certificates will not be substituted by staff.
- Online course participants must have access to the Internet, a device with audio and video capabilities (computer, smartphone, tablet, etc.), and an email address.
- Upon successfully completing the course, the Participant will be issued a Certificate of Completion immediately online, and where applicable, course information will be sent electronically.
- For NYS Offices of Teaching Initiatives, we automatically report the information of all successful course completers to the state every week. It may take 24-72 hours for the data to appear in your Lux account after reporting.
- For NYS Office of the Professions, Documentation in the form of an authorized Certification of Completion must be submitted to the State Education Department at the time of reregistration or initial application for licensure, certification, or a limited permit. (Issued as PDF immediately upon completing course.) Submitting a Certification of Completion Form for NYS Offices of the Professions:

- Applicants for New York State Licensure can upload a completed form on the Office of the Professions’ website at https://eservices.nysed.gov/professions/addl-doc/.
- Current Licensees can upload a completed form on the Office of the Professions’ website at https://eservices.nysed.gov/professions/wf/licensee-doc-upload/.

- REQUESTED REFUNDS are subject to a $2 refund fee. No refunds will be issued after 1 hour from registration and payment. This is a 2 hour training.
THE COURSE MUST BE COMPLETED WITHIN SIX (6) WEEKS OF PAYMENT AND REGISTRATION. IF THE COURSE IS NOT COMPLETED WITHIN SIX (6) WEEKS OF PAYMENT AND REGISTRATION, THE COURSE WILL BE DEREGISTERED AND ALL COURSE PROGRESS WILL BE FORFEITED. RE-REGISTRATION FOR THE COURSE WILL REQUIRE A NEW, SEPARATE PAYMENT.
PAYMENT IS REQUIRED IN FULL PRIOR TO TAKING THE COURSE. IF CHARGES ARE REVERSED OR PAYMENT IS NOT RECEIVED, YOUR COURSE PROGRESS WILL BE DELETED AND YOUR CERTIFICATE WILL BE RENDERED INVALID.
